One of the most common misconceptions in regenerative medicine is that Platelet-Rich Plasma (PRP) contains stem cells.
This misunderstanding is easy to see. Both treatments involve injections, both aim to support the body’s natural healing process, and both are commonly used for orthopedic injuries, tendon problems, and osteoarthritis. Because PRP is prepared from a patient’s own blood, many people assume the concentrated platelets are actually stem cells.
In reality, PRP and stem cell therapy are two different treatments that work through different biological mechanisms.

What Is PRP?
Platelet-Rich Plasma, or PRP, is created by drawing a small sample of the patient’s blood and processing it in a centrifuge. This separates the blood into different layers, allowing the platelet-rich portion to be collected for injection.
Platelets are best known for helping blood clot after an injury, but they also contain numerous growth factors and signaling proteins involved in tissue healing.
When PRP is injected into an injured tendon, ligament, muscle, or joint, these growth factors are released into the surrounding tissue and may help support the body’s natural repair response.
Importantly, PRP does not contain mesenchymal stem cells under routine preparation methods.
Does PRP Contain Stem Cells?
For standard PRP prepared from peripheral blood, the answer is no.
Peripheral blood contains very few circulating mesenchymal stem cells under normal conditions. The centrifugation process used to produce PRP concentrates platelets, not stem cells.
This means that although PRP contains platelets, plasma proteins, cytokines, and growth factors, it should not be described as “your own stem cells.”
Patients sometimes confuse PRP with bone marrow concentrate, also known as BMAC, because bone marrow contains stem and progenitor cells. These are different procedures with different biological compositions.
What Are Umbilical Cord Derived Mesenchymal Stem Cells?
Umbilical Cord Derived Mesenchymal Stem Cells (UC-MSCs) are living cells isolated from donated umbilical cord tissue following healthy full-term deliveries with informed maternal consent.
Unlike PRP, which primarily delivers concentrated platelets and growth factors, UC-MSCs are living cells that communicate with surrounding tissues through biological signaling.
Rather than directly replacing damaged tissue, current evidence suggests that much of their activity occurs through paracrine signaling. They release cytokines, growth factors, extracellular vesicles, and other signaling molecules that help regulate inflammation and influence the body’s own repair processes.
These cells generally do not remain permanently in the body, and responses vary depending on the condition being treated and individual patient factors.
How Do PRP and Stem Cell Therapy Differ?
Although both treatments fall under the broad umbrella of regenerative medicine, they are fundamentally different.
PRP is a blood-derived product prepared from the patient’s own circulation and is designed to concentrate platelets and their associated growth factors.
Stem cell therapy uses living cells that possess different biological characteristics and communicate with the body’s tissues in more complex ways.
Neither treatment should be viewed as universally better than the other. The most appropriate option depends on the patient’s diagnosis, the severity of the condition, treatment goals, age, previous treatments, and the physician’s clinical assessment.
In some situations, PRP may be an appropriate first regenerative option. In others, mesenchymal stem cell therapy may be considered depending on the clinical indication and available evidence.
Why Do So Many People Think PRP Contains Stem Cells?
Part of the confusion comes from the fact that both treatments are often discussed together and are used to treat similar musculoskeletal conditions.
Marketing language has also contributed to this misunderstanding. Terms such as “natural healing cells” or “your body’s repair cells” are sometimes used to describe PRP, leading patients to believe that platelets and stem cells are the same thing.
While both treatments participate in tissue repair, they do so through different biological mechanisms.
Using the correct terminology helps patients better understand what they are receiving and supports informed decision making.
Can PRP and Stem Cell Therapy Be Used Together?
In some clinical settings, physicians may combine regenerative therapies as part of an individualized treatment plan.
Whether combining treatments provides additional benefit depends on the condition being treated and remains an area of ongoing research. At present, there is no single combination protocol that has been shown to be appropriate for every patient.
Treatment decisions should always be based on the patient’s diagnosis, current evidence, and physician assessment rather than assuming that combining therapies is automatically better.
